ORDER OF SERVICE: Plaintiff's request for pro bono counsel is denied without prejudice. The Court dismisses Plaintiff's claims against New York City Department of Correction. The Clerk of Court is directed to add the City of New York as a Defendant under Fed. R. Civ. P. 21. The Clerk of Court is directed to notify the New York City Department of Correction and the New York City Law Department of this order. The Court requests that the City of New York waive service of summons. The Clerk of Court is instructed to send Plaintiff one U.S. Marshals Service Process Receipt and Return form for Defendants Corizon and New York City Health and Hospitals Corporation so they may be served as provided. The Clerk of Court shall serve a copy of this order and the complaint on the New York City Law Department at: 100 Church Street New York, New York 10007. An Amended Civil Rights Complaint form is attached to this order. The Court certifies, pursuant to 28 U.S.C. ' 1915(a)(3), that any appeal from this order would not be taken in good faith, and therefore in forma pauperis status is denied for the purpose of an appeal. See Coppedge v.
