JUDGMENT as to Eric Saldarriaga (1), Pleaded guilty to Count(s) 1, The defendant is hereby committed to the custody of the United States Bureau of Prisons to be imprisoned for a total tem1 of: 3 months. The court makes the following recommendations to the Bureau of Prisons: Due to Mr. Saldarriaga's significant health issues and need for ongoing medical monitoring and intervention, the Court recommends that he be designated to a BOP CARE level 2 facility near his home in Queens (which will also allow for family visitation), such as FCI Fairton, FCI Fort Dix, FCI Danbury or FCI Otisville. The defendant shall surrender for service of sentence at the institution designated by the Bureau of Prisons: before 2 p.m. on 8/25/15. Upon release from imprisonment, the defendant shall be on supervised release for a term of: 3 years, the first 3 months of which shall be on home confinement (with the conditions set forth on page 4). For the first three months of supervised release, the defendant shall be on home confinement with Location Monitoring, which program may include electronic monitoring or voice identification. During this period of home confinement, the defendant may leave his home only to work, attend school, participate in religious services, attend medical appointments for himself and his immediate family, and take his children to and from school, and for such other activities approved by the Probation Office. During this time, the defendant will maintain a telephone at the defendant's place of residence without call forwarding, a modem, caller ID, call waiting, or portable cordless telephones for the above period. Location Monitoring shall commence on a date to be determined by the probation officer. The defendant shall pay the costs of Location Monitoring on a self-payment or co-payment basis as directed by the probation officer. The defendant shall provide the probation officer with access to any requested financial information. The defendant shall not incur new credit charges or open additional lines of credit without the approval of the probation officer unless the defendant is in compliance with the installment payment schedule. The defendant shall submit his person, residence, place of business, vehicle, or any other premises under his control to asearch on the basis that the probation officer has reasonable belief that contraband or evidence of a violation of the conditions of the release may be found. The search must be conducted at a reasonable time and in reasonable manner. Failure to submit to a search may be grounds for revocation. The defendant shall inform any other residents that the premises may be subject to search pursuant to this condition. The defendant is to report to the nearest Probation Office within 24 hours of release from custody. The defendant shall be supervised by the district of his residence. The defendant must pay the total criminal monetary penalties, $100 special assessment, $1000 fine. Payment during the term of supervised release will commence within 30 (e.g.. 30 or 60 days) after release from imprisonment. The court will set the payment plan based on an assessment of the defendant"s ability to pay at that time. The defendant shall forfeit the defendant's interest in the following property to the United States: See separately docketed forfeiture order. (Signed by Judge Richard J.
